How to use this tool
Keep the two-week timeframe consistent and choose 0–3 for every item. Complete all questions and note major life changes separately when comparing records from different periods.
Results and examples
The 0–24 total and editorial bands can help start a conversation. A high value does not establish disease, and a low value does not prove that support is unnecessary.
Worked example
Choosing 1 for each item produces 8 points, meaning simply the sum of eight responses.
Limitations
- These original items have no validated reliability or equivalence to a recognised scale.
- Persistent difficulty or impaired functioning can warrant support regardless of score.
